A report into health and safety procedures at the Department of Psychiatry, Letterkenny General Hospital has shown a number of issues of major concern.
Patients in the hospital’s psychiatric unit had to endure a leaking wall and toilets without a flush handle.
The inspection of the psychiatric unit found the leak led to dampness and chipping paint.
The inspectors also discovered a child was inappropriately admitted to the adult unit.
The young patient was secure but there were no activities suitable to their age.
The inspection carried out in June last.
Ligature points were discovered despite high dependency patients being cared for in the unit, the Mental Health Commission report said.
Even though a cleaning schedule was in place there was a strong smell of urine from two toilets, it was also reported.
